As the parent or guardian of the player named above, I acknowledge that participation in athletic events carries the risk of physical injury. I understand that my child will be engaging in strenuous physical activity, and I take full responsibility for ensuring that my child is physically capable of participating, with no known medical conditions that would prevent them from doing so.
In consideration of accepting my child's participation in the Inner Chi Basketball Organization's activities, I hereby release, discharge, and hold harmless the organization's coaches, board members, tryout volunteers, and the operators, employees, and owners of the facilities from any claims related to physical injury that may occur to the player named below while participating in the activities. This release includes any injury caused by the negligence of any organization representative, volunteer, or coach while carrying out their duties during the Inner Chi Basketball Organization's tryouts, practices or training sessions.
I also understand that if my child requires medical attention, I, or my designated emergency contact listed below, will be notified. In the event of an emergency where I or my emergency contact cannot be reached, I authorize the attending representatives of Inner Chi Dragons Basketball Organization to secure appropriate medical treatment for my child.
